Day of set-up, you may unload your materials at your booth location; however, all vehicles must be moved to the designated parking area immediately after drop-off. Parking by booths is prohibited.

I, on behalf of myself, or on behalf of my minor child, (hereinafter "participants") fully understand that participation in any recreational, training, or simulation activity or activities (hereinafter “activity”) offered by the Rancho Cucamonga Fire Protection District (hereinafter “Fire District”) exposes participants to the risk of property damage or personal injury, including, but not limited to, injuries to the muscles, back, knees, wrists, hands, fingers, feet, neck, or other physical injuries, up to and including death. I hereby acknowledge that participants are voluntarily participating in this activity and are aware of and agree to assume any such risks arising from such participation. 

I, on behalf of participants, heirs, administrators, executors and assigns, hereby release, discharge and agree not to pursue any claims, demands, actions, or suits, and to indemnify and hold harmless, the Fire District and the City of Rancho Cucamonga (hereinafter “City”), it's elected officials, officers, agents, and employees, for any injury, death, or damage to, or loss of personal property arising out of, or in connection with, participation in the activity from whatever cause, including claims involving the active or passive negligence of the Fire District and/or the City or any other participants in such activity. 

I, on behalf of participants, consent to emergency medical treatment and transportation to a medical care facility in the event of any injury, which in the professional assessment of the Fire District, requires emergency medical treatment and/or transportation. 

I, on behalf of participants, consent to the taking of photographs and/or video during the activity by the Fire District, the City, and/or any contractor hired by the Fire District or the City and understand that photographs and/or video may be used in Fire District and/or City publications and/or websites without liability or compensation of any sort. I HAVE CAREFULLY READ THIS RELEASE, HOLD HARMLESS, AGREEMENT NOT TO SUE, AND CONSENT FOR MEDICAL TREATMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT IT IS A FULL RELEASE OF ALL LIABILITY AND SIGN IT ON MY OWN FREE WILL.
